This scholarship is for applicants who are accepted or enrolled in a program of study in the fields of either Occupational or Physical Therapy.
Application deadlines are February 15 and August 15 each year. |

These scholarships are awarded to students who have been accepted or are attending an accredited school of occupational therapy (including art, music, or physical therapy). Applicants must be US citizens.
All applicants must obtain a letter of sponsorship from their local DAR chapter. Your state scholarship chairman will assist in locating the chapter nearest you. Please send a stamped self addressed business size envelope to obtain the name and address of your state scholarship chairman and an application for any of the following scholarships.

Only applications completed correctly and submitted in one package will be considered. Incorrect applications or applications mailed to the incorrect address are discarded. No records are returned. Scholarships are awarded without regard to race, religion, sex or national origin. Do not submit a personal photo. If your records feature a photo, please cover it! Only winners are notified by the National DAR Scholarship Chairman following the approval of a ruling by the Executive Committee of NSDAR.
